How do I avoid last-minute conflict with physicians over discharge orders?

One way to avoid such discharge problems from the beginning is to have the physician communicate the plan for discharge at the time of admission or on the day of surgery.  For example, hospital postoperative order forms may be changed so the first line reads: "Anticipate discharge to ___________________. Estimated length of stay_______days."
